is TRT medication or therapy?

Would you class testosterone replacement therapy as (1) medication or (2) therapy ( as the name suggests!)
This treatment is to raise the levels of a person with low testosterone to normal levels same as everyone else.
testosterone is a natural hormone found in all healthy animals.
( depending on your definition its not a drug then)

ps, would a blood transfusion be seen as medication??? ( same sort of lines?)

I’m really sorry but both medication and therapy are essentially the same.
To medicate is just ‘to treat medically’, and hence ‘medication’. That is to treat by medical science, and not necessarily by the narrow term to treat with a medicine or medicament.
Therapy, is ‘curative medical treatment’. Again this does not necessarily mean with a medicine

Testosterone replacement therapy could just as well be called Testosterone replacement medication.

With regard to a blood transfusion, again it could be referred to as either. I totally accept that in most people’s mind, a blood transfusion is not thought of as being given a medicine, and it isn’t. However it is a medical treatment, and so fits either of the above terms.
Unfortunately common usage is often at variance with the formal dictionary definition, and of course the language and its usage is continually evolving. This is where confusions arise, and makes your question entirely valid.
